Application to Travel Form (ATF) Instructions

OSA "On-Line" Forms for Tournaments. http://www.ontariosoccer.ca/forms.htm
ATF: Pasted below is the process for Team Officials, Clubs and the District Association.
a. Applicants email ONLY their APPLICATION NUMBER to CLUB to request processing:
b. Club Official pulls up the respective APPLICATION on-line via Administration to review the PENDING Application in the Database and emails/forwards ONLY the ID NUMBER (TAF#, etc.) to District Association to request Approval and/or Change.
NOTE: The ID NUMBER (TAF#, etc.) is preferred in the rare case of duplicate Application Numbers. However, the Application Number will also work.
c. District Administrator approves the request and notifies both the Applicant and the OSA. The DA will invoice the club. Please note that copies of approved ATF's are not required for Ontario Tournaments as the approval is linked directly to the Tournament Host TAF and they have access to the complete list of approved.
ATF's for those teams traveling to an out-of-province tournament a copy will be sent either by e-mail to the club.
d. The OSA updates Applications and Approvals Databases weekdays after 4pm. Approved Applications are listed on-line after the update is completed.
